Office Policies
Confidentiality
Be assured that our office staff is trained to keep patient information confidential in compliance with HIPAA. No information about you or your treatment will ever be released without your written authorization. In turn, we ask that you respect other patient confidentially by not discussing if you see someone in our office.
Insurance
As a courtesy, our office will file claims with your insurance carrier. We are not responsible for collection or negotiating disputed claims. Please note, it is patient responsibility to notify the front desk of updated polices. If you do not provide the front desk with an updated and active policy before your service, you will be responsible for what is not covered or denied.

Appointments
Your appointment time is a specific period of time reserved for you. If you wish to cancel, we ask that you please notify our office 24 hours in advance. If you do not notify our office, you’ll be marked as a no show. We do have a 15 minute grace period, but please be aware if you arrive after this 15 minutes you will need to reschedule. If you are a minor, please note you will need a parent, legal guardian, or approved adult at the time of your appointment.
Payment
Patients must pay their copayments before services are rendered. Specially, you are responsible for what your insurance does cover. If you do not have insurance, you are responsible for your self pay establish visit cost prior to services, and any add on (i.e. swabs, injections, EKG, Urine Cultures, etc.) you must pay at check-out.
Prescription Refills
If you are in need of a prescription refill, we ask that you please request this at the time of your appointment. If you are in need of a refill in between appointments, please contact your pharmacy directly first. If they are not able to help you, please call our office and choose the nurse of your provider and leave a detailed message. to avoid running out of medication, please notify our office 24-48 before your refill is due.
Paperwork
If you are needing medical paperwork filled out by your provider, you will need to make an appointment specially for this to be completed. If time does not permit, our providers have up to 10 business days to complete the paperwork and return to you. This includes, but not limited to, disability forms, physical forms, and insurance forms.
